Careful
planning has gone into developing an employee benefits package which will
provide a broad foundation of security for our employees and their families.
The benefits described below are provided at no cost to full-time employees
(except for a nominal dental contribution), and most benefits begin after
your first thirty days of employment.
Please note that this is only a summary of the Company's employee benefits as of January 2009. 1. Medical Coverage is provided to employees and their eligible dependents at NO COST to the employee. Integral Vision participates with Blue Cross Blue Shield's Community Blue PPO. The Medical Plan provides the option to participate in a PPO (Preferred Provider Organization) allowing for increased coverage. 2. Dental Coverage is provided by Delta Dental of Michigan and is available for a nominal contribution. Delta Dental offers participation in a PPO, which allows for a higher degree of coverage. 3. Life/AD&D Benefits Life insurance benefit equal to one times annual base salary, excluding commissions, overtime, bonuses or any other extra compensation. In the event of accidental death, your beneficiary receives an additional one times your annual base salary. 4. Education Assistance (Undergraduate) is available to employees after 6 months of full-time employment. Tuition, books and required fees are reimbursed at the rate of 100% for successful completion ("C" grade or better) for any college-accredited course. Maximum semester benefit is $1,000 and maximum calendar year benefit is $5,000. 5. Flexible Spending Accounts allow employees to set aside money on a pre-tax basis to cover health and dependent care expenses not covered by insurance. 6. Disability Plans, both short and long term provide pay for illness, injury or pregnancy. 7. Paid Holidays & Vacation - The company recognizes the following holidays: Memorial Day, Fourth of July (2 days), Labor Day, Thanksgiving and the Friday following, and Christmas Day through New Years Day. Vacation time is accrued on a current, calendar-year basis, so you are able to take vacation in the same year it is earned. Vacation time will vary depending on the hire date. 8. 401(k) Employee Savings Plan An optional tax-deferred retirement plan, employees may defer between 1 and 15% of their compensation. The Company will match 20% of your deferral, up to 6% of your salary. Salary deferral, the Company match and any earnings are immediately 100% vested.
